D4 form to be completed by a medical Doctor and they will charge you for that service.
D2 it's for you to complete. You need Categories C or C+E as provisional entitlements they all give you the same entitlements.
Stage: 2
Once all this is done post D4, D2 forms together with your driving licence to DVLA address given on the forms.
Stage: 3
Studying and passing LGV theory test, and hazard perception, CPC Module 2
